- All applicants are screened
to determine qualifications and competence as described in the
current standards of the Joint Commission on Accreditation of
Healthcare Organization (JHACO), and the guidelines for Use of
Supplemental Nursing Services as published by the American Nurses
Association Commission on Nursing Services.
- Current and original copy of Licenses or Certificate
- Current and original copy of Driver's License
and/or Photo Identification
- Social Security Card
- Current and original CPR card
- Current and original Fire Life Safety Training
Card if you reside or work in L.A County
- Proof of compliance with applicable immigration
laws and maintenance of I-9 documentation
- Current health history, including a record of
immunizations (MMR & Varicella), current PPD skin test or
chest x-ray (if applicable), and physical examination
- A minimum of two professional references of past
employment
- Successful completion of
pre-employment 12 panel drug screening
- Clearance of appropriate criminal background check
at a minimum of a state level
- Clearance of Office of Inspector General (OIG)
and General Services Administration (GSA)
- Recent CEU's
- Resume (if applicable)
